Membership Application

After browsing our web site and checking out our “About Us” and “FAQ” pages, we hope you will see the benefits of becoming a member in our organization. If you wish to join our organization, please refer to the following list of general information which includes how to apply for membership and the application process:

  1. You may request a membership application form be mailed or emailed to you by contacting us via email at our LAFFIA email address listed below on this page.

  2. If the email request is for an application to be mailed to you via the U.S. Postal Service please be sure to include your name and address to include zip code.If the email request is for an application to be emailed to you, please be sure to include your name and email address. Note: Application form which will be attached to our email response was created with Microsoft Word 2003.

  3. Please include some general information in your request such as your work organization or business and job title. Membership in our organization must meet the general criteria as outlined on the “About Us” and “FAQ” pages of the web site. We do not send applications out on a simple request with no other information provided.

  4. Once the application has been completed please return to the completed form to us at our mailing address which is included on the application form. No dues or fees are required at the time the application is submitted.

  5. Once your application has been received and reviewed you will be notifed via email or U.S. mail that you have been approved or declined for membership.

  6. If your membership request has been approved, the email or letter will request you submit your dues payment to LAFFIA at that time. Upon receipt of your dues payment you will be notified and your membership will become active. Mailing address of LAFFIA will be included in this confirmation notice.


SPECIAL NOTICE:

  • If you are being sponsored for membership by a current LAFFIA member please be sure to put that information on the designated space provided on the application form. Persons sponsored by a current LAFFIA member are normally accepted for membership once the sponsoring LAFFIA member has been contacted and the new applicant’s information has been verified by the sponsoring LAFFIA member.

  • If you are not being sponsored for membership by a current LAFFIA member, the submission of the membership application to LAFFIA will be deemed as permission from you for LAFFIA to contact your organization or business to verify your employment and job position. This is done to determine that the information you provided is valid and that you meet the general membership criteria. Only verification of your employment and job title will be made ... No other inquiries will be made as to your work history, etc.
